Healthcare monitoring system (HCMS) uses remote patient monitoring technology enabling patients with severe chronic diseases or conditions to monitor their blood pressure and other health factors from the comfort of their homes and share this information electronically with their physicians and other healthcare providers.
Tracking the patients’ results on a regular basis, the healthcare team can adjust treatments as required. In the longer term, HCMS provides guidance to patients in order to help them learn to live with their life-long condition.
Patients with chronic conditions such as Hypertension disease, chronic obstructive pulmonary disease (COPD). Heartbeat instability and some other health conditions can benefit from a healthcare monitoring program leveraging remote patient monitoring technology.
Once the patient is referred to the program by a healthcare professional in the hospital or at a primary care facility, they are introduced to a clinical care team that will track and adjust the execution of the care plan as well as provide support and guidance for the patient.
